About the Role: As a Clinical Care Coordinator, you will play a vital role in ensuring that patient care plans are effectively coordinated among various healthcare providers.
What You'll Be Doing: - Coordinating patient care plans by liaising with all necessary care providers and medical professionals to ensure patient needs are met. - Using computer software to admit and discharge patients, coordinate follow-up appointments, and refer patients to specialists as needed. - Masterminding ways to improve the quality of patient care provided by the facility. - Creating efficient systems and workflows within the clinic. - Attending departmental meetings and staying up to date on all business initiatives related to the clinic. - Collecting and inputting patient data, insurance information, and financial information into the computer system.
Our Ideal Candidate Has: - A degree in MBBS. - Experience in clinical care coordination or a related field is preferred. - Strong communication and organizational skills.
